Case Study
Primary Care Practice
Meet Pelican
Pelican Family Medicine is a full-service family practice, with two locations in Wilmington, NC, serving nearly 4,000 patient lives.
They specialize in family medicine, allopathic medicine, and osteopathic medicine, which includes geriatric care, pediatric visits, and ongoing adult primary care for many chronic conditions.
The Challenge
Like many small to medium sized primary care practices, keeping up with mandated testing and reporting was a challenge.
On the front end, there was no simple way to engage with their patient population to identify testing and screenings that each patient was eligible for. This resulted in poorer patient outcomes, missed revenue opportunities, and potential MIPS penalties.
Additionally, they did not have all the programs in place to offer these services to patients, nor did they have the staff to manage everything.
The Strategy
Pelican engaged with Fairway Health & Wellness to solve this issue. This included:
• Electronic Patient Engagement to complete HRA’s which allowed for billing an E&M billing code
• Pair the HRAs with billable ancillary services identified, each requiring little to no staff time
• Complete patient enrollment into appropriate ancillary services
Next, Pelican implemented the Precision Platform, which automates the process of uncovering required tests and screenings for each patient.
-
- Fairway created a Group Practice in Precision Platform
- Data was analyzed for each practice group including all patients for all payers using Precision’s proprietary technology
- The Group Practice was white labeled, specific email and text communication was approved and sent
- Precision’s Virtual engagement created a specific e-visit for each patient
- Engagement included email interaction and direction with the patient for follow-up with the Group Practice practice group and any additional assessments that were may be required based the patient’s HRA score and physician approved protocols.
